a pouch above the band that has a small gastriccapacity that limits the amount of oral intake people can take. and we have obviously ourgold stantard procedure, which is a gastric bypass and the gastric bypass is both a restrictiveand a malabsorptive procedure. and a third procedure is a relatively new procedure itscalled the vertical sleeve gastrectomy. this is a purely restrictive procedure in whichthe stomach is reduced in capacity. you're also taking out an area of the stomach calleda fundus where it produces a hormone called ghrelin and that hormone is essentially ahunger hormone that works on the brain for hunger. so actually taking out a hunger hormone.gastric bypasses are technically difficult surgeries; they are done laparoscopicallyand as a result of laparoscopy that really
has revolutionized weight loss surgery forthe morbidly obese patients. post operative pain is minimized, operative stress is minimized.recovery is quick. dr. awad explains who would be good candidate for surgical weight loss.essentially patients with a body mass index of 35 with one or more weight related comorbiditiesare considered surgical candidates for weight loss. or individuals who have a body massindex of 40 without weight related comorbidities. surgical weight loss really produces profoundweight loss with profound effects on their health and it is durable and sustainable.we have a lot of long term data showing this. the surgical weight loss procedures offermechanical restriction and physiologic changes that really facilitate weight loss and it'snot only the individuals self control and
self discipline, they actually have a mechanicalrestriction and physiologic changes that promote this weight loss. but the after care is veryimportant. exercise, dietary changes and implementing appropriate nutritional supplementation ingood quality diet is very important to success. what i tell all my patients is that regardlesswhat they have done its better than not having anything done. because even modest weightloss has profound effects on your health. so essentially we tailor the individual thesurgery based on the individuals operative risks their realest expectation and theirrisk tolerance because some patients are concerned about the fact that their stomach is cut andtheir intestines are rerouted and they want something relatively simple like a band. otherpatients want more drastic weight and they
